Treatment Plan

The following is a treatment plan between Children's Division and Marty Lambeth. The purpose of this agreement is to outline for Mrs, Lambeth the minimal expectations of the Children's Division and the Greene County Juvenile Court. Completion of the following responsibilities will indicate the desire on behalf of Mrs. Lambeth to regain custody of her child, Ben Jacob Lambeth.

RKSPONSIBLILITES OF MARTY LAMBETH:

1. Mrs. Lambeth agrees to cooperate with the Children's Division. This includes informing her social worker of any changes in address, employment or household composition before the change occurs.

2. Mrs. Lambelh agrees to sign all release of information forms requested for any and all professionals/agencies involved in determining her compliance with the conditions of her treatment plan.

3. Mrs. Laambeth will provide and maintain a stable place of residence which meets minimal health standards.

4. Mrs. Lambeth will not allow or reside with individuals in the household who would pose a threat to her child or who have a known history of, or conviction for any activities involving physical, emotional, 0r sexual abuse of children.

5. Mrs. Lambeth will obtain a psychological evaluation by a professional approved by the Children's Division social worker, Deputy Juvenile Officer, and the Guardian Ad Litem. Mrs. Lambeth will follow all recommendation made by the mental health professional.

6. Mrs. Lambeth will participate in individual and family counseling by a professional approved by the Children's Division worker. Deputy Juvenile Officer, and the Guardian Ad Litum

7. Mrs. Lambeth will participate in anger management classes through an agency approved by Ihe Children's Division worker. Deputy Juvenile Officer, and the Guardian Ad Litem.

8. Mrs. Lambeth will participate in a sexual offender's evaluation
by a professional approved by the Children's Division worker,
Deputy Juvenile Officer and the Guardian Ad Litem.

10. Mrs. Lambeth agrees to schedule visitation with Ben Jacob. If the Children's Division worker, the Deputy Juvenile Officer, and the Guardian Ad Litem agree that visitation is detrimental to the child, visitation will be suspended until such time as the matter is brought before the court.

11. Mrs. Lambeth realizes that failure to complete the treatment plan may result in a recommendation of the Children's Division to the Greene County Juvenile Court for a petition to be filed to terminate her parental rights.

RESPONSIBILITBS OF CHILDREN'S DIVISION:

1. The Children's Division worker will make referrals to appropriate resources to assist Mrs. Lambeth in meeting her responsibilities. If funds are available, Children's Division will help pay for these services.

2. The Children's Division worker will provide updates to all team, members.

The Treatment Plan entered into between Marty Lambeth and the Children's Division shall be in effect until such time as a new plan has been approved by the court. Failure to comply with the conditions and terms of this plan may result in the filing of a petition to terminate parental rights.

ORDER .
NOW UPON THE 25day of January, 2005, the Court took up the oral motion of the Counsel for the Children’s Division requesting permission to cut the hair of the minor child. The Court deferred ruling upon the motion until February 1, 2005, to allow the parents the time to consult with their counsel regarding the signing the signing of appropriate treatment plans in this
case.

The Court carefully considered the arguments of parents' counsel presented mat die cutting of female hair was prohibited by the chosen religion of the parents. However, the Court also considered the testimony of Mindy Ellis, LCSW, who testified that die minor child's hair was of such excessive length that it was difficult to care for, was unhealthy, and adversely affects the child's self-esteem. The Court does hereby sustain the Children's Division's oral motion and hereby authorizes the Children's Division to cut the minor child's hair.
